Corporate Kidz Australia – Parent/Guardian Consent and Liability Release

Neural Information Processing Systems (NeurIPS) Conference Children’s Program

Liability Release

We, the undersigned parent(s) or legal guardian(s), agree to enrol our child/children in the Corporate Kidz Australia Children’s Program conducted in conjunction with the Neural Information Processing Systems (NeurIPS) Conference.

For ourselves, our child/children, and each of our respective heirs, executors, administrators and assigns, we release and agree to indemnify and hold harmless Corporate Kidz Australia, Neural Information Processing Systems (NeurIPS), the conference organisers, venue owners and operators, and their respective officers, directors, employees, contractors, volunteers, agents and representatives (collectively referred to as "the Released Parties") from any and all claims, liabilities, demands, actions, damages, losses or expenses arising from or relating to our child’s participation in the Children’s Program.

Nothing in this release excludes liability arising from the gross negligence, wilful misconduct, or any liability that cannot be excluded under applicable law.

Medical Consent

In the event of illness, injury or any medical emergency involving my child, I authorise Corporate Kidz Australia to:

• Administer appropriate first aid.
• Contact my nominated emergency contact and/or medical practitioner.
• Arrange ambulance transport or emergency medical treatment where considered necessary.

I acknowledge that I am responsible for any medical or associated costs incurred in relation to my child’s treatment.

Participation

I acknowledge that activities provided within the Children’s Program are designed to be age-appropriate and supervised by Corporate Kidz Australia staff. I consent to my child’s participation in activities and games selected at the professional discretion of Corporate Kidz Australia.

I understand that Corporate Kidz Australia reserves the right to refuse or discontinue a child’s participation where their behaviour may disrupt the program or compromise the safety or wellbeing of themselves, other children or staff.

Parent/Guardian Declaration

I declare that:

• The information provided on my child’s registration form is true and correct.
• I have read and understood this Consent and Liability Release.
• I voluntarily agree to the terms contained within this document.
• I acknowledge that participation in the Children’s Program is subject to these conditions.
• I agree that I will remain on the premises of the International Convention Centre 14 Darling Drive, Sydney, NSW 2000 Australia at all times while my child(ren) are in the care of Corporate Kidz Australia.
